How to Schedule Accomodated Exams with Examination Services
Step One: Make sure you have arranged academic accommodation with a Services for Students with Disabilities (SSD) counselor.
Note: You must speak to an SSD Counsellor for EACH course for which you are registered. Every time you add a new course, you need to contact your SSD counsellor. If you log in and do not see all the courses for which you would like accommodation, you MUST contact your SSD counsellor.

SSD is located in Room 4110 of the Western Student Services Building. SSD Kings is located In the Wemple Building, Room W157 for King's students. If you have already arranged academic accommodation with SSD you may proceed to Step Two.
Step Two: Log In to the Examination Services and SSD King's Login page to provide exam or test dates.
Once logged in, you will see your name at the top of the screen and your courses listed at the bottom of the page. To submit a request for a test or exam, select the course from the drop down menu and press submit.

This will take you to an online form where you can submit test and exam dates for that course. Once you have submitted an exam date, you will receive a confirmation email, and when you return to the main page, you will see the exam date and time listed in green.
You MUST submit a request for EACH test or exam that you want to arrange with Examination Services. At the start of each term, you should refer to your course outlines to find out dates and times for tests in your courses and submit a request for each test or exam to Exam Services. If you don’t know the dates of your tests or exams, ask your instructor.
If you can’t sign up on line, you can fill out an Exam Request form (Exam Request Form (.pdf) or King's Exam Request Form (.pdf) ), and hand this into Student Central Room 1120 Western Student Services Building or Wemple Building Room W160 for King's courses.
While it is best to submit all your requests as soon as you have your course outlines, you MUST submit requests at least 14 days in advance of the test/exam date.
For exams that are scheduled by the Office of the Registrar (December and April exams), you submit requests after the exam schedule is posted.
- The deadline to submit requests for December exams is: November 18th, 2012.
- The deadline to submit requests for April exams is: March 17th, 2013.
The screen you will see is sampled below. For any exams you wish to write with Accommodated exams simply click the yes button and this will active your sign up for that specific exam.

Step Three: Check your quiz/test/exam Dates, Times and Locations.
One week before the date of the exam, your location and start time will be displayed AT THE TOP OF THE SCREEN. In all cases you should verify the date ,time and location of your exam the day before your exam is scheduled in case there have been any changes made from when it was first posted.

If you are unable to access your exam schedule online, please call (519) 661- 2100, or go to Student Central, Room 1120 Western Student Services Building. King's students please call 519-433-0041 Ext. 4568 or go to Wemple Building Room W160.
Step Four: On the Day of Quiz/Test Exam.
Arrive at the appropriate location 15 minutes before the scheduled start time for your exam. See here for maps of the UWO campus:
Bring your Western ONECard for identification purposes (or another valid piece of government issued Photo ID).
Making changes quiz/test/exam schedules
To change information from a previously submitted Exam Request Form; complete an Exam Change form. The Exam Change form can be picked up in Student Central, Room 1120 Western Student Services Building or Wemple Building Room W160 at King's. Note: this form requires the signature of your Dean's Office (Academic Counsellor) and the signature of your instructor.
To change the regular date/time (when class writes) of quiz/test/exam, complete an Exam Change form and have your professor approve the change by signing the form.
