Course Enrollment/Records

Visiting Student Application Form (pdf)

This application is used to allow Visiting Students to apply to take courses at Western for transfer to a degree program at another University.

Visiting Undergraduate Student (Research Only)

Visiting Undergraduate Students (Research only) are allowed to conduct research at Western University but do not fall under an approved exchange program. Students are registered as Part-Time Visiting Undergraduate Students on the condition that they cannot take courses at Western.

Letter of Permission Form (pdf)

The Letter of Permission form is used to grant approval to take courses at other universities for transfer credits. Details on the application process are on the Letter of Permission page.

Add/Drop Form (pdf)

This Add/Drop form is used during the Paper Add/Drop periods. More information is available on the Add/Drop/Swap page.

Payment for Third Party Forms (pdf)

This payment form is used for making payments for Third Party Forms, and is to accompany any third party form that is mailed or faxed into Student Central for completion.

Diploma Replacement Form (pdf)

This form is used to obtain replacement diplomas. Details about this process can be found on the Diplomas page.

Name Change Request Form (pdf)

This form is used by students to change their name for official registration purposes. Details on this process can be viewed on the Updating Personal Info page.

2016-2017 Academic Year

Listed below are all the forms for the 2016-2017 academic year. Use these forms if your program starts anytime between August 1, 2016 to July 31, 2017.

Summer OSAP Graduate Students (pdf)

This is the Summer Ontario Student Assistance Program (OSAP) Application for Graduate Students.

Summer OSAP Undergraduate Students (pdf)

This is the Summer Ontario Student Assistance Program (OSAP) Application for Undergraduate Students.

Summer OSAP Change Form (pdf)

This form is used for indicating to your online Ontario Student Assistance Program (OSAP) Application that will require reassessment.

OSAP Change Form (Fall/Winter)  (pdf)

This form is used for indicating changes to your online Ontario Student Assistance Program (OSAP) Application that will require reassessment.

2016-17 Supplemental Information Form for Western Students Participating in International Exchange Program  (pdf)

This form is used to provide exchange study period start and end dates for your government funding application; and/or have your Official Western Letter or Third Party Form request processed to include the dates you are on exchange.

Change of Income Form  (pdf)

This form is used when a change in your income is going to impact your previously submitted Ontario Student Assistance Program (OSAP) application.

Academic Edit Letter (pdf)

The Academic Edit Letter is used during the Ontario Student Assistance Program (OSAP) process if an explanation is required regarding your Academic Progress.

Academic Progress Letter (pdf)

The Academic Progress Letter is used during the Ontario Student Assistance Program (OSAP) process if an explanation is required regarding your Academic Progress.

Permanent Disability Certification Form (pdf)

This form is used to obtain disability-related information from your physician or other regulated health care practitioner. This is required for your Ontario Student Assistance Program (OSAP) application if you're applying as a student with a permanent disability.

Studies at Another Institution Form  (pdf)

This form is used to indicate if you're taking courses at another institution of study that will be credited to your University of Western Ontario degree, to help in your Ontario Student Assistance Program (OSAP) assessment.

Letter of Direction/Mailing Authorization Form  (pdf)

This form is used to authorize Student Financial Services to mail my Ontario Student Assistance Program (OSAP) for Part-Time students funding.

OSAP Consent to Release Information Form  (pdf)

The OSAP Consent to Release Information Form is used to allow the release of your Ontario Student Assistance Program (OSAP) information to persons other than yourself.

Continuation of Interest-Free Status/Confirmation of Enrolment (Schedule 2)  (pdf)

Form to maintain interest-free status on previous loans if not receiving OSAP for current study period.
